Interdisciplinary Team
Health Care Professionals
Working together, our interdisciplinary team works with families to develop a plan of care that is right for each patient’s individual situation and needs.
Physicians
Our Medical Director works with your primary care physician or in some cases will become your primary care physician if your doctor is not able to follow you to hospice.
Nurse Case Managers
Family Hospice nurses are experts at managing the needs of each patient. They educate caregivers about how to care for their loved one. These specially trained, experienced nurses develop and manage an individualized plan of care for each patient and provide compassionate support.
Hospice Aides
Certified nursing assistants (CNAs) provide personal care and help patients with activities of daily living. They also provide companionship and valuable emotional support.
Social Workers
Social workers coordinate resources and help the patient and family with non-medical concerns. A social worker can comfort and give support to family members, help them plan for the future, assist them in making financial arrangements and ease other emotional difficulties, including grief and bereavement.
Chaplains
Chaplains help patients and families understand and cope with spiritual issues and concerns. Family Hospice staff can work with families directly or can coordinate services with the patient’s and families church leaders or spiritual counselors.
Volunteers
Volunteers provide companionship and emotional support and can help patients and families in a variety of ways.
