Patient Rights


Family Hospice protects and promotes the rights of each individual under our care, including each of the following rights:

The right to be fully informed in advance about the care and treatment provided by the agency.

The right to be informed of the agency’s policies on Advance Directives, including a description of applicable state law.

The right to voice grievances with respect to treatment or care.

The right to confidentiality of clinical records.

The right to have one’s property treated with respect.

The right to be fully informed orally and in writing of all items and services furnished the agency for which payment may be made; the coverage available for such items and services under Federal Programs of which the agency is reasonably aware and any charges for items and services not covered.

The right to be fully informed in writing of a patient’s rights and obligations.


The right to be informed of the availability of the State Hospice Agency Hotline.
